In order to register a student at Ridgewood Middle School you will need the following items:
1.    Proof of residency in the form of a current utility bill or rental agreement in your 
       name.  If you are living with someone in their home, we will need a letter from the 
       homeowner stating that you and your student are residing at the home and the letter
       must be notarized.  You must bring the notarized letter and a copy of the home-
       owners utility bill.
2.    Current immunization records.
3.    A Birth Certificate is helpful but not required for registration.
4.    If your student has an IEP, we will need a copy of the current IEP and a copy
       of the current Diagnostic report in order to properly place your student in the
       correct classes.
5.    A copy of the student's last report card is also helpful but not required for
6.    All previous schools for the student will be contacted regarding discipline at
       time of registration.
